Use this form to inform your tipped employees about how the tip credit is applied to minimum wage calculation as mandated by the U.S. Dept. of Labor.

The U.S. Department of Labor recently amended its tip credit notice regulations, effective May 5, 2011, to require employers to inform tipped employees of certain tip credit information. This template has been designed to help operators meet compliance requirements.
